For insurance eligibility please follow the instructions below: 

1. Verify your eligibility. Pre-approval is needed for coverage. 

2. Fill out the information and submit.

3. You will receive an email from Lactation Network with information regarding your eligibility for 100% coverage.  This email is frequently sent within hours of your submission.

4. Once you receive verification from Lactation Network, book an appointment and choose an available time for your visit.
